We need volunteers to assist in our Transitions Services program. We assist individuals with transitioning out of a nursing home or rehabilitation setting into the community to live independently.
Job Description:
Transitions Assistant – provide support to the Transitions Coordinator by assisting with transitioning individuals out of nursing facilities
Shop with the consumer and Transitions Coordinator; locations are based upon the needs of the consumer
Push a grocery cart and assist in selecting items needed to live independently
Ability to follow and maintain a budget while shopping
Ability to delineate between household, cleaning products, personal care, and grocery items and keep the items in their category for ease at check out
Transport items purchased to consumer’s new residence
Periodically assist in putting items away, as needed by consumer
May assist with moving heavier items, depending on the needs of the consumer
Research low cost home goods / beds / items for consumers
Assist Transitions Coordinator in trips to the store, City Utilities, Habitat for Humanity, or other locations to locate necessary items for the consumer
Ability to effectively communicate with others via face-to-face and text messages
